National Case Management Week is an important time to highlight the crucial work case managers do across healthcare settings. At CompleteOK, our case managers are an essential part of our Home Health, Hospice, and Home Services (Oklahoma Advantage Program & VA Program), ensuring patients receive the care and resources they need to improve their quality of life. Their work is multifaceted, involving coordination of care, patient advocacy, education, and resource management. Here’s a closer look at what our case managers do across our services.


Case Management in Home Services: Oklahoma Advantage Program

The Oklahoma Advantage Program provides in-home services to individuals who qualify for Medicaid long-term care benefits. In this role, case managers at CompleteOK focus on helping patients remain independent in their homes, preventing the need for institutionalized care. Case managers assess the needs of patients and work closely with family members and caregivers to develop individualized care plans. These plans often include services such as personal care, homemaker assistance, and respite care, which are tailored to ensure patients can safely live at home while maintaining their dignity.


In the Oklahoma Advantage Program, case managers also help patients access community resources and support services, ranging from meal programs to transportation assistance. They work as patient advocates, ensuring that those in need receive the full spectrum of care and benefits they’re entitled to, and coordinate services with caregivers to ensure everything runs smoothly.



Case Management in Home Health

In CompleteOK Home Health, case managers play a vital role in coordinating care for patients who require medical services in the home following illness, surgery, or injury. They act as a bridge between the patient, healthcare providers, and family members, ensuring that care is seamlessly integrated and focused on recovery.


Case managers assess the patient’s medical and personal needs, create care plans, and arrange for services such as skilled nursing, physical therapy, occupational therapy, and more. They work to ensure that the home health services align with the patient's recovery goals and that all healthcare needs are met. One of their primary responsibilities is patient education, making sure that both the patient and their family understand the care plan, medications, and the steps necessary for a successful recovery.


Additionally, case managers help to coordinate follow-up visits, monitor the patient's progress, and adjust care plans as needed. Case Management in Hospice Care
