Patients are selected based on their ability to meet eligibility for services: a physician certifying the need for prescribed services to include (Skilled Nursing, Physical Therapy, Speech Therapy, Occupational Therapy, Social Services, Home Health Aide, and other ancillary services).  The patient may be required to be homebound, depending on coverage requirements.
